BRAZOSPORT COLLEGE COMMUNITY HEALTH NETWORK HEALTH CENTER

Starting next week, Tuesday, March 2nd, Brazosport坳lege CHN Health Center will be open from 1:00 p.m. – 6 p.m. on Tuesdays for BC faculty, staff and students with behavioral health and medical providers available for face-to-face services. The CHN Health Center is located in the B Wing of BC Central and can only be accessed from the courtyard near Tommy’s Grill and the BC Bookstore. Virtual care is also available by visitinghttps://mychn.org/services/virtual-care/. All appointments can be scheduled by calling (281) 824-1480 or by visitingwww.mychn.org.A pre-registration form is available athttps://portal.mychn.org/enroll/request_enroll.php. The Brazosport College CHN Health Center is an outpatient healthcare facility that provides a variety of services, including adult medicine, chronic care management, behavioral health and women’s health, among other services. CHN accepts most private insurances, Medicaid, Medicare, and CHIP. Uninsured individuals may be eligible for discounted self-pay services based on their family size and household income. CHN strives to provide the highest value of care and therefore offers substantial benefits for people who are uninsured and self-pay.

COMMUNITY COLLEGE SURVEY OF STUDENT ENGAGEMENT (CCSSE)

This spring, Brazosport College will continue its participation in a national survey focused on teaching, learning, and retention in community colleges, the Community College Survey of Student Engagement (CCSSE). The project is part of the Center for Community College Student Engagement and the Community College Leadership Program at The University of Texas at Austin.

In the past, the survey was administered to students in randomly selected classes using a paper survey. This year the survey will be administered online to all credit students, 18 years and older.

The student survey runs from February 24, 2021 to March 18, 2021. The faculty survey runs from March 15, 2021 to April 2, 2021. Please look for more information coming soon to your email inbox.

Brazosport College is intent on being a leader in higher education, and this survey can assist us in improving course completion rates, as well as the rate of student persistence to the completion of their educational goals. ONLINE COVID-19 DAILY HEALTH SCREENING PROCESS

Brazosport College health screening process is online for all employees, students, and visitors who come to campus.你需要填写问卷每个day you intend on coming to the Brazosport College campus.The COVID-19 Health Screening questionnaire can be accessed inEnglishorSpanishby visitingwww.www.d-onis.com/screening. This questionnaire has been designed to be quick and simple to fill out, and can be easily accessed through all internet browsers, smartphones, and tablets.

Once completed, you will be shown either a green check mark with “APPROVED FOR ENTRY” if you have passed, or a red exclamation mark with “STAY HOME, STAY SAFE” if you failed, as well as your name, and the date you completed the questionnaire.

Anyone that does not pass the COVID-19 Health Screening questionnaire should not come to campus. Everyone that passes should be prepared to show the confirmation email.

Brazosport College has increased focus on safety monitoring to promote safety protocols of social distancing, wearing a face mask/covering, practicing good hygiene, etc. Once on campus, have either the confirmation email or confirmation screen of your completed questionnaire available as you may be asked at random to show proof of completing the self-screening tool.

Upon arrival to campus, you will be able to access all parking lots and enter campus through most entrances. To provide clarification, face masks/coverings are required for all students, employees, and visitors (ages 10 and up) any time on campus.To clarify, face shields are not considered face masks/coverings. If an individual chooses to wear a face shield, the individual must also wear a face mask/covering which fully covers the mouth and nose except as allowed in the following exceptions.The exceptions are: if you are walking, running, or biking on the trails; if you are in your vehicle; if you are alone in your office with the door closed; if you are a welding student actively welding and wearing a welding face shield; and, if you are an instructor lecturing students, you may wear a face shield instead of a face mask in addition to maintaining 6ft. of distance. Otherwise, all individuals, students, faculty, staff, grounds keepers and contractors are to wear a face mask/covering.

For reference, the CDC defines “close contact” as someone who was within 6 feet of an infected person for a cumulative total of 15 minutes or more over a 24-hour period starting from 2 days before illness onset (or, for asymptomatic patients, 2 days prior to test specimen collection) until the time the patient is isolated.

With the rise in positive COVID-19 cases across the country, medical experts are reinforcing the importance of use of masks and social distancing.Please continue following the CDC guidelines for prevention of the COVID-19 Coronavirus, which includes, but not limited to, thorough hand washing, social distancing and the wearing of face masks or face coverings.

Please note, the COVID-19 Contact/Exposure Report Form is separate and apart from the Health Screening Questionnaire. The COVID-19 Contact/Exposure Form should be completed if you have been exposed to someone or if you have tested positive for COVID-19.
